Patient Forms
We understand that our patients are very busy. That’s why we have included our patient forms on our website, so you can fill them out before your appointment and save time in the office. Please bring the forms, along with your insurance information, to your first appointment!
Scheduling an Appointment
If you must cancel your appointment, please give us a call at 704-739-7588 with at least 24-hours notice so we can offer this time to another patient. We can reschedule your appointment if needed.
Please arrive at your appointment 10 minutes early in case we need to complete any paperwork. We do our best to see you at your scheduled time, but we apologize in advance if there is a delay due to an unexpected emergency. If you arrive more than 15 minutes late to your appointment, you may have to be rescheduled.
If you miss three appointments without 24-hours notice, we reserve the right to charge a $50 cancellation fee per missed appointment.

Billing & Payment
Payment Information
We require payment in full at the time services are rendered. We accept cash, check, Visa, MasterCard, and Discover. We thank you for your cooperation. Additionally, we offer CareCredit financing for those who may require more flexible payment options. CareCredit is a healthcare credit card that allows you to finance your dental treatments with low monthly payments and no upfront costs.
Insurance
If you have dental insurance, it is very important for you to understand your plan’s benefits, deductibles, and exclusions, as all plans are different. You can learn your plan’s benefits by contacting your dental insurance company before your visit with us. We are happy to discuss your treatment plan with you so you have the appropriate information to give your insurance company. We are here to help you receive your maximum allowable benefit, but you are responsible for any deductibles, co-pays, and remaining balances. We will submit dental claims to your insurance provider. However, if your provider does not cover the claims, you will be responsible for the charges.
